Desk/Review Request Form 

 

 

To order text copies for review or for a desk copy once an adoption has been made, please fill out and submit the Online Form below or contact Klista Love toll free at 1-800-543-0874 (ext. 2207).  Click here if you would prefer to fax or mail your request.

Complimentary books are sent at no charge by regular parcel post and could take 7-12 business days to receive. If you would like to pay for UPS shipping to expedite your request please note “Ship regular UPS” in the Message Box at the bottom of the form. We will then include an invoice just for the UPS shipping charges with the book(s) and you should receive your book(s) by UPS in 4-7 business days.
